入会時に必要なもの
- 1.入会申込書（要記入、捺印） 18歳未満の方は本人以外に保護者の署名、捺印も必要です ※道場にて配付致します。
- 2.入会金10,800円
- 3.月会費2ヶ月分前納
- 4.口座振替依頼書（要記入、捺印） ※記入用紙は道場にて配付致します。
- 5.銀行・郵便局など金融機関への届け印
- 6.道衣10,800円〜21,600円（税込）、プロテクター4,500円〜（税込）、施設管理費2,700円

入会規約
レッスン料・オプションサービスの納入
- 1.月謝は毎月前月の27日にご指定の口座よりお振替となります。
- 2.当道場が必要と判断した場合、レッスン料は変更される場合があります。
- 3.コース変更は、月謝が減額となるものは前月10日までに申請していただき適用となります。 （月途中、前月10日過ぎの申請による返金は致しません） また、月謝が増額となる変更は当月に現金で差額をお支払いいただき適用となります。
退会・休会
- 1.退会する場合は、前月の10日までに申し出てください。それ以降の申請は、翌月分の月謝やオプションサービス料を清算していただいての退会となります。
- 2.休会する場合は、前月の10日までに申し出てください。休会費用は毎月2,160円となります。
- 3.一度退会をされてからの再入会にあたっては、改めて入会金がかかります。
欠席やクラスの変更・振替について
- 1.2週間以上にわたる欠席の予定の際には、ご連絡ください。
- 2.レッスンは講師の急な都合や災害などにより、中止、または日程の変更をする場合がございますが、その際の補償は致しません。 年末年始、連休、お盆、メンテナンス等でのお休みは随時ご連絡いたします。
- 3.欠席の振替は、その月内であれば自由におこなうことができます。ただし最終週の急な欠席については、次月の第1週までに消化することができます。
その他
- 1.レッスン中はインストラクターの指示に従い、他の人の迷惑になる行為は慎んでください。
- 2.許可を得ずにレッスンを撮影、録画、録音することはできません。
- 3.住所や連絡先など、届け出事項に変更がある場合は、所定の用紙に記入して、届け出をしていただきます。
- 4.メンバーとしてふさわしくないと当会により判断された場合には、その対象者の主観とは別に退会をしていただくことがあります。 その際には入会金や月謝、道衣代等の返金は一切おこないません。
Rules
Lesson fee and optional service
- 1.Lesson fee will be debited from your bank account on every 27th of the month.
- 2.Lesson fee can be changed when it is necessary.
- 3.When you change a lesson course and lesson fee is lowered, you must inform Dojo before 15th of the previous month to get a refund. When you change a lesson course and lesson fee is increased, you must pay the difference in the month.
Quite / Recess
- 1.When you quit Dojo, you must inform Dojo before 10th of the previous month. If you inform Dojo after 10th of the month, next month lesson fee and other fee will be charged.
- 2.When you recess the lessons, you must inform Dojo before 10th of the previous month. Monthly fee will be 2,160 yen per month.
- 3.If you reenroll the lessons, admission fee will be charged again.
Absence / change lesson or substitute lessons
- 1.Please inform us when you absent lessons more than 2 weeks.
- 2.Lessons can be canceled or re-scheduled in case of instructors’ circumstances or natural disaster without compensation. End of the year, national holidays or maintenance days will be announced.
- 3.You can take missed lessons in the same month. When you need to absent lessons in the last week of the month, you can take them in the 1st week of the next month.
Other
- 1.Follow instructions of Sensei and refrain from bothering other members.
- 2.Recording lessons are not allowed without permission.
- 3.Fill in the form when your address and contact info are changed.
- 4.If you are judged not suitable as a member, we may ask you to leave Dojo. In this case, there is no refund for any lesson fee, uniform cost, and etc.
